The only thing bad, is the spine. The logo should be bigger.

#4, The pic on the back is a hi-def still from the game. I have tried to make the cover like a movie cover and the pic I chose doesn't give anything away about the game.
#5, I suppose this is down to personal preference?

Pretty nice box. Nice color scheme. I just don't like grenades on the back. They look too random..
