#6, well the story is that the front is mainly an image, to be honest. But I extended it to reach to the bottom of the box and removed a logo in the corner. But the back is all custom. All lines were drawn in by myself.
Oh, I forgot: credit to Silent Oblivion (I think) for the screen borders.
